What is it?

Anagen effluvium, is an interruption in the hair growth process or the anagen phase (Growing phase). It usually occurs in patients receiving radiation therapy or chemotherapy, where scalp hair falls out rapidly.

The resting or telogen phase is usually intact, so hair grows back after 3 to 6 months. This type of alopecia is non-scarring and therefore reversible.

Causes

The most common causes of Anagen effluvium are:

  • Radiotherapy

  • Chemotherapy, especially those prescribed at high doses and in combinations. Alkylating agents such as doxorubicin, nitrous oxide, and cyclophosphamide are particularly important.

  • Other medications that inhibit cell growth include bleomycin, dactinomycin, daunorubicin, fluorouracil, and high doses of methotrexate.

  • Certain medications at very high doses, such as colchicine and cyclosporine (although the latter very rarely causes hair loss, but rather promotes some hair growth).

  • Poisons such as thallium, arsenic, gold, or bismuth.

Clinical Signs

Hair loss is rapid and extensive over a few days and occurs diffusely throughout the scalp.

Diagnosis

The diagnosis is clinical and based on the patient’s information and history. The Pull test and Rebora test are markedly positive. Trichoscopy reveals black dots, yellow dots, Pohl Pinkus constrictions, tapering hairs, and tulip-shaped hairs.

Anagen effluvium is the sudden loss of hair during the anagen (growth) phase of the follicular cycle.
It involves the loss of up to 80% of hair in a short period of time, no more than a month.
