What is it?

Antiandrogens prevent DHT from triggering hair miniaturization, thus preventing androgenetic alopecia.

There are several antiandrogens used in medicine. They are classified as steroidal, such as spironolatone, also used for acne, or nonsteroidal, such as bicalutamide. Bicalutamide is an antiandrogen used in the treatment of female androgenetic alopecia (FAGA). Its effictiveness has been demonstrated in a study published in 2020 by Ismail et al.

These drugs were initially developed for the treatment of prostate cancer and have subsequently been applied to other androgen-induced conditions such as hirsutism, acne, hormonal acne, and androgenization syndrome.

They are used orally, as well as in the form of mesotherapy or microinjections, With this being a much more specific and effective route of administration

What does the procedure involve?

Since the basic mechanism by which androgenetic alopecia develops is the interaction of dihydrotestosterone with its receptors in the dermal papilla cells and hair follicles, antiandrogens block these receptors so that, even if dihydrotestosterone is produced, the hair miniaturization effect cannot be triggered and alopecia does not develop.

Receptor blockage is a very effective treatment for female androgenetic alopecia, but it cannot be used orally in male patients because it would interfere with other androgen functions. For this reason, mesotherapy, where this type of medication is administered only locally, is a much safer method than oral administration.

Several androgen receptor blockers have been used for the treatment of female androgenetic alopecia, such as flutamide, bicalutamide, and spironolactone. Of these, bicalutamide is the one used in mesotherapy due to its long half-life and high avidity for androgen receptors.

What is a session like?

The microinjection or mesotherapy procedure is performed with microneedles to reduce pain.

How often are sessions necessary?

The frequency of microinjections or mesotherapy sessions ranges from once a week to several months, depending on the indication.

When will I see results?

New hair usually appears after the third month, as the hair’s resting period on the scalp is three months.

Who is a good candidate?

Patients suffering from alopecia areata are excellent candidates for treatment with microinjections of anti-inflammatory drugs. It is well known that applying an anti-inflammatory drug locally to the plaque allows for at most 1% penetration of the drug, while injections are much more effective.

A comparative study showed that injecting corticosteroids into alopecia areata plaques significantly shortens the recovery period compared to applying topical corticosteroids, which have virtually no effect on the progression of the disease.

Contraindications

The procedure should not be performed on people with serious concomitant conditions, such as those undergoing active cancer treatment or those with fever, serious infection, or similar conditions. For complete details on contraindications, consult your physician.

Pre- and/or Post-Treatment Advice

  • Before undergoing treatment, it is important that the patient doesn’t come in on a day when they are feeling unwell or are not in a condition to receive microinjections. It is not advisable to undergo the treatment on a day when they have a headache, the flu, or are generally unwell.

  • After treatment, it is important not to wash your hair that same day and wait until the next day to use the appropriate shampoos. One of the most suitable shampoos to use concomitantly with antiandrogens is one containing ketoconazole. It should be used so that it is in contact with the scalp for at least five minutes, and massage thoroughly.

  • Occasionally, some patients experience some pain or swelling in the scalp treated with mesotherapy. This is a self-limiting reaction that resolves spontaneously, but it can cause concerns for some patients. It does not require treatment, but anti-inflammatory medications can be optionally taken for one or two days.

Scientific Evidence

 

There are 15 scientific articles on the effects of antiandrogens such as bicalutamide on the scalp.

Bicalutamide mesotherapy is a very novel treatment that has proven to be very effective and for which there is statistically significant evidence (Gomez-Zubiaur A, Andres-Lencina JJ, Cabezas V, Corredera C, di Brisco F, Ferrer B, Rodriguez-Villa A, Subiabre-Ferrer D, Valenzuela C, Diez DV, Ricart JM. PMID: 37305191; PMCID: PMC10251290). Bicalutamide has also been injected into the scalp with the aid of radiofrequency (Pegalajar-García MD, Gil-Villalba A, de la Torre-Gomar FJ. There are other antiandrogens used in Alopecia such as Spironolactone, of which more than 112 studies have been published (Wang C, Du Y, Bi L, Lin X, Zhao M, Fan W.
