Mesotherapy with Dutasteride or Finasteride.

Effective and painless.

What is it?

Dutasteride and Finasteride are 5-alpha reductase inhibitors, the enzyme that converts testosterone into dihydrotestosterone (DHT). It is this latter hormone that triggers the hair follicle miniaturization effect and androgenetic alopecia in both men and women.

Dihydrotestosterone (DHT) is an essential hormone only in childhood for male growth and development. However, in adulthood, it increases prostate size, the risk of benign prostatic hyperplasia and prostatic adenocarcinoma, the appearance of atheromatous plaques in arteries, high blood pressure, ischemic cardiomyopathy, diabetes, obesity, and metabolic syndrome. For this reason, the metabolic pathway of its synthesis can be inhibited through 5-alpha reductase inhibitors. These inhibitors, such as Dutasteride and Finasteride, can be given orally, although they are more specific and 30 times higher concentrations are reached if they are infiltrated directly into the scalp (6). This has been demonstrated by Saceda Corralo, Rodrigues Barata and Vaño et al. in a study published in 2017 in Int J Trichology (7).

What does the procedure involve?

5-alpha reductase inhibitors such as Dutasteride and Finasteride prevent the formation of a hormone crucial to the development of androgenetic alopecia. This hormone, called 5-alpha reductase, converts testosterone into dihydrotestosterone within the cells of the pilosebaceous follicle and hair papilla.

The dihydrotestosterone hormone then binds to androgen receptors, which interact with specific receptors in our genes, triggering the hair miniaturization process.

What is a session like?

3. PRODECIDE

The microinjection or mesotherapy procedure is performed with microneedles to reduce pain.

4. MEDICATION

Small doses of medication are placed in the areas with hair loss.

5. MINIMIZING DISCOMFORT

The sessions are performed with the assistance of a vibrating device to minimize discomfort.

6. RETURN TO YOUR ROUTINE

You can return to your normal routine immediately after the procedure.

How often are the sessions necessary?

The frequency of mesotherapy sessions with Dutasteride or Finasteride ranges from once a week to several months, depending on the indication.

The first results after mesotherapy treatment sessions with Dutasteride and Finasteride are visible after the 4th session.

Who is a good candidate?

The best candidates for mesotherapy treatment with Dutasteride or Finasteride are people suffering from androgenetic alopecia, particularly men. The main mechanism by which men develop androgenetic alopecia is through the interaction of dihydrotestosterone and the receptors present in follicular stem cells.

In women suffering from androgenetic alopecia, the amount of dihydrotestosterone present in the cells is much lower, as women are able to utilize testosterone through other metabolic pathways and do not convert it into dihydrotestosterone as easily as men. However, women experiencing female androgenetic alopecia often desire localized administration of 5-alpha reductase inhibitor drugs through mesotherapy rather than orally. This way, they achieve a more local and targeted effect on the scalp.

Contraindications

Pregnant women cannot receive injections of 5-alpha reductase inhibitors, nor can they take these medications orally. This could affect fetal development if the child they are pregnant with is a boy.

Pre- and/or Post-Treatment Advice

  • Before undergoing treatment, it is important that the patient does not come in on a day when they are feeling unwell or are not in a condition to receive microinjections. It is not advisable to undergo treatment on a day when they have a headache, flu symptoms, or feeling generally unwell.

  • After treatment, it is important not to wash your hair that same day and wait until the next day to use the appropriate shampoos. One of the most suitable shampoos to use concomitantly with 5-alpha reductase inhibitors is one containing saw palmetto. It should be used so that it is in contact with the scalp for at least five minutes, and massage thoroughly.

  • Occasionally, some patients experience some pain or swelling in the scalp treated with mesotherapy. It is a self-limiting reaction that resolves spontaneously, but it can be worrying for some patients. It does not require treatment, but anti-inflammatory drugs can be taken optionally for one or two days.

Scientific Evidence

 

There is considerable evidence on the effect of mesotherapy with Dutasteride or Finasteride on androgenetic alopecia.

In 2017, the first cases of microinjection or mesotherapy with Dutasteride for the treatment of Androgenetic Alopecia were published, with surprisingly positive results (Saceda-Corralo D, Rodrigues-Barata AR, Vañó-Galván S, Jaén-Olasolo P. Mesotherapy with Dutasteride in the Treatment of Androgenetic Alopecia. Int J Trichology. 2017 Jul-Sep; 9(3):143-145. doi: 10.4103/ijt.ijt_73_16. PMID: 28932074; PMCID: PMC5596657.).
Dutasteride, a 5-alpha reductase inhibitor, also appears to be a neuroprotective, antioxidant, and anti-inflammatory drug that could even be used in the treatment of amyotrophic lateral sclerosis (Proaño B, Casani-Cubel J, Benlloch M, Rodriguez-Mateos A, Navarro-Illana E, Lajara-Romance JM, de la Rubia Ortí JE. Gupta et al. (Gupta AK, Polla Ravi S, Wang T, Talukder M, Starace M, Piraccini BM. Systematic review of mesotherapy: a novel avenue for the treatment of hair loss. J Dermatolog Treat. 2023 Dec;34(1):2245084. doi: 10.1080/09546634.2023.2245084. PMID: 37558233.) reviewed more than 400 published studies reporting the positive effect of mesotherapy on androgenetic alopecia: Dutasteride and 5-alpha reductase inhibitors are the most commonly used compounds.
Mesotherapy or microinjections with 5-alpha reductase inhibitors are also very safe and allow patients who cannot take oral medication to treat their androgenetic alopecia. In a series of 541 patients (Saceda-Corralo D, Moustafa F, Moreno-Arrones Ó, Jaén-Olasolo P, Vañó-Galván S, Camacho F. Mesotherapy With Dutasteride for Androgenetic Alopecia: A Retrospective Study in Real Clinical Practice. J Drugs Dermatol. 2022 Jul 1;21(7):742-747. doi: 10.36849/JDD.6610. PMID: 35816059.), it was concluded that mesotherapy with dutasteride produces very few and limited side effects.
