What it is?

Triamcinolone is an anti-inflammatory drug from the corticosteroid family that acts locally and has very little systemic absorption. It is used to prevent inflammatory processes such as the formation of excessive scarring or keloids, alopecia areata, frontal fibrosing alopecia, and all inflammatory hair disorders in general.

A comparative study conducted in patients with alopecia areata plaques showed that the duration of these plaques was significantly reduced when they were injected with triamcinolone weekly or biweekly, while the application of topical corticosteroids had no effect on the progression of the disease.

What does the procedure involve?

Several alopecia processes involve inflammation. Some of these processes induce scarring and destroy the hair follicle. This inflammatory process must be stopped to prevent the alopecia from progressing.

Microinjections of anti-inflammatory drugs such as triamcinolone and others are reserved for inflammatory processes of the hair shaft. They are especially relevant when this inflammatory process leads to scarring.

Patients with scarring alopecia, where hair follicles are completely destroyed, cannot expect to recover the hair lost over the previous years. The goal of treatment is to stop the progression, but not to recover hair that has been destroyed and no longer exists.

The anti-inflammatory drugs used in these processes are primarily corticosteroids. Oral administration of corticosteroids induces various side effects such as water retention or weight gain. Microinjections with anti-inflammatory drugs are especially indicated precisely to prevent these side effects.

What is a session like?

How often are the sessions necessary?

The frequency of microinjection or mesotherapy sessions ranges from once a week to several months, depending on the indication.

When will I see results?

The results after mesotherapy with anti-inflammatories depend on the degree of alopecia, but are usually visible after the 4th session.

Who is a good candidate?

Patients suffering from alopecia areata are excellent candidates for treatment with anti-inflammatory microinjections. It is well known that applying an anti-inflammatory drug locally to the plaque allows for at most 1% penetration of the drug, while the injection is much more effective. A comparative study showed that injecting corticosteroids into alopecia areata plaques significantly shortens the recovery period compared to applying topical corticosteroids, which have virtually no effect on the progression of the disease.

Other ideal candidates for anti-inflammatory microinjections are people suffering from frontal fibrosing alopecia or lichen planus. In these patients, the inflammatory process permanently destroys the hair, and the goal of anti-inflammatory drugs is to halt this immune process against the hair follicle to prevent it from progressing further and destroying other hair follicles.

Contraindications

The procedure should not be performed on people with serious concomitant conditions, such as those undergoing active cancer treatment or those with fever, serious infection, or similar conditions. For complete details on contraindications, consult your physician.

Pre- and/or Post-Treatment Advice

  • Before undergoing treatment, it is important that the patient not come in on a day when they are feeling unwell or are not in a condition to receive microinjections. It is not advisable to undergo treatment on a day when they have a headache, the flu, or are feeling generally unwell.

  • Given the small surface area of inflammatory alopecia, low doses of anti-inflammatory drugs are required, and microinjections usually only cover a few square centimeters of the entire scalp. For this reason, significant edema or pain reactions are extremely rare.

Scientific Evidence

 

Microinjection of anti-inflammatory drugs such as triamcinolone is the treatment of choice for inflammatory alopecia.

It is the first-line treatment for frontal fibrosing alopecia (Iorizzo M, Tosti A. Frontal Fibrosing Alopecia: An Update on Pathogenesis, Diagnosis, and Treatment.
