The postoperative period is crucial for proper graft integration and minimizing complications.
In this article, we discuss the most important medical aspects of the postoperative period, recommended cosmetic products, and behaviors to avoid to ensure an optimal and long-lasting result. During the first few days after transplantation, the scalp is in an acute inflammatory state. Grafts are delicate structures that require a controlled environment to survive, vascularize, and grow.

Therefore, it is essential to follow certain evidence-based medical guidelines
Gentle and gradual washing: During the first and second days, it is recommended to avoid direct contact with water on the recipient area. Washing should be done with a mixture of water and shampoo, applied with gentle tapping motions using a sterile sponge or clean fingertips.
From the fifth to the seventh day, washing can begin with moderate pressure and very gentle massage. This protocol helps prevent the formation of scabs, which can interfere with follicle growth.
Local hydration and anti-inflammation: The use of saline solutions or thermal water spray promotes hydration of the graft and improves the metabolic environment for cell regeneration, in addition to relieving the itching that may occur as part of the healing process.
Strict hygiene and minimal contact: During the first 10 days, touching the recipient area, wearing tight-fitting caps, or scratching due to itching should be avoided. Any trauma could dislodge the implanted follicles. It’s recommended to sleep with your head elevated for the first few days to reduce swelling and avoid direct contact with contaminated surfaces.
Another essential post-transplant care option is choosing the right postoperative cosmetics. Not all shampoos and lotions on the market are formulated for a newly operated scalp.
Medical hair cosmetics are based on ingredients that cleanse without irritation, soothe the skin, and promote skin regeneration.
Clascoterone is a topical According to scientific evidence, the use of shampoos with gentle, sulfate-free and alcohol-free formulas and a physiological pH is recommended for at least the first two weeks postoperatively.
Key ingredients include panthenol, which moisturizes and improves the skin barrier; allantoin, which has a soothing and regenerative effect; and aloe vera, which helps reduce itching and inflammation.
While there are active ingredients beneficial for recovery, there are also components that should be avoided, such as menthol, artificial fragrances, concentrated essential oils, and keratolytics like salicylic acid, as they could irritate or inflame the treated area.
From the third week onward, if the progress is favorable, with no signs of infection, most scabs having fallen off, and the skin having re-epithelialized, the use of active topical products may be authorized.
This phase coincides with the end of the inflammatory phase and the beginning of the follicle activation process.
During this stage, lotions containing active ingredients such as liposomal minoxidil, topical finasteride, caffeine, and biotin can be used. These ingredients all have properties that stimulate the follicle’s anagen phase and promote hair growth.
However, even the best products lose effectiveness if combined with procedures that compromise the follicular integration phase. It is essential to keep in mind that, between day 1 to 14, the grafted follicles go through a stage called the ischemia-revascularization phase, during which they establish their new connection with the recipient capillary bed. This phase is critical, and certain actions can jeopardize the success of the procedure.
The following describes some of the behaviors that should be avoided.
Intense physical exercise or excessive sweating: Increased temperature and sweat can promote infection and impair healing.
Wearing helmets, tight hats, or dirty caps: These can cause friction on the grafts and dislodge them, in addition to increasing the risk of infection.
Smoking or consuming alcohol: Tobacco causes peripheral vasoconstriction, reducing oxygenation to the graft. Alcohol increases edema and can interfere with healing.
Direct sun exposure: During the first 3-4 weeks, direct sunlight on the transplanted scalp should be avoided. Ultraviolet radiation can induce inflammation and hyperpigmentation of the scarred areas.
Swimming pools, saunas, or hot tubs: These are contraindicated during the first 3 weeks due to the risk of contamination and chemical irritation.
In conclusion, it is important to understand that the success of a hair transplant does not end with the surgery.
